Complimentary Beginning and Advanced Diagnostic Decoding Surveys
Our complimentary Diagnostic Decoding Surveys are an easy and efficient way to detect when students are having word-level reading difficulties, and pinpoint their specific decoding weaknesses.
Our two most popular surveys are the Beginning and Advanced Decoding Surveys:
The Beginning Decoding Survey assesses how well students are reading words they should have mastered by the end of first grade:
- High frequency words
- Single syllable words with short vowels, digraphs, and two-sound blends
The Advanced Decoding Survey assesses the decoding skills students should have mastered by the end of third grade:
- Single syllable words with short vowels and more advanced phonics patterns (e.g. trigraphs, three-sound blends)
- Single syllable words with advanced vowel patterns
- Multisyllable words (real and nonsense)
The complimentary packet contains everything you’ll need for initial assessment of an unlimited number of students. The packet also provides an explanation of how the survey results can be used to group students, and it offers guidance on how to plan effective and efficient instruction.

This complimentary set of assessments includes informal diagnostic tools designed to quickly assess pre-decoding skills in emerging readers. These assessments are commonly used in Kindergarten and 1st grade, and with older students who perform well below expectations on basic reading tests. The Pre-Decoding Skills Survey are not yet aligned with the Grouping Matrix. However, it is an important first step in identifying students in emerging readers for early remediation.
This download includes the Advanced Decoding Survey Plus (ADS Plus). This quick diagnostic assesses a student’s ability to read the complex words necessary to comprehend content area text. It allows educators to refine their understanding of students who are exhibiting difficulties with multi-syllable words or words with advanced vowels. The results can be entered into the Grouping Matrix™ to more effectively target students’ specific decoding issues. Commonly used in grades 4-12.
Phonics Blitz™ is part of Phonics Suite. It is a set of 50 intervention lessons for grades 4-12 and is designed for students with mild, moderate or specific decoding deficits. Blitz can be taught by a wide range of educators. The instruction teaches students how to accurately read complex multisyllable words, and quickly helps them become more confident and effective readers. It is commonly used in RtI and ELL intervention settings for grades 4-12.
Phonics Boost ™ is part of Phonics Suite. It is a set of 80 intervention lessons for students in grades 2–12. Boost is designed for older students with significant or severe decoding deficits or for students in grades 2–3 who are experiencing difficulties acquiring phonics knowledge. Boost instruction teaches students how to accurately read basic words yet quickly moves on to complex multisyllable words. It is commonly used in RtI, Special Education, and ELL intervention settings for grades 2-12.
